The report, Review of all fatal accidents in Queensland mines and quarries from 2000 to 2019, by Dr Sean Brady, was tabled in Queensland Parliament in February 2020.
It is an independent analysis of the fatalities, serious accidents and high potential incidents that have occurred in the Queensland mining and quarrying industry over the past 20 years.
To reduce the rate of fatalities, serious accidents and high potential incidents, requires significant change within the industry. In this webinar, Dr Brady will discuss the four key areas of the 11 recommendations offered in the report and the basis for their recommendation.

Dr Sean Brady | Managing Director of Brady Heywood.
Dr Sean Brady is a forensic structural engineer who specialises in identifying the causes of structural engineering failures. Sean has investigated structural collapses both nationally and internationally, and has acted as an expert witness on numerous occasions. He is a Chartered Professional Engineer, a Fellow of Engineers Australia, a Board member of the Society of Construction Law Australia, and a member of the Panel of Experts for the Singapore International Mediation Centre.
