A fire occurred on a water truck located at the top of a ramp.

A fire occurred on a water truck located at the top of a ramp. The operator stopped the truck and activated the fire suppression system. The fire suppression system was unsuccessful in extinguishing the fire. Hand held extinguishers and a water cart were used to extinguish the fire. No one was injured. An escape of fluid onto a hot surface was identified as the cause.

Fly rock incident
During a blast, fly rock was ejected approximately 300m, beyond the exclusion zone, and into an area where personnel were thought to be in the safe zone. 

The blast was in an area of the quarry that had not been blasted for several years and it was prepared using proven design parameters previously used at the quarry. This included an exclusion zone of 80m, which was calculated from the design burden.

Farmer loses his life undertaking dam repair works
WorkSafe is investigating an incident in which a dry dam collapsed in on a man undertaking works at its base. The 28-year-old was working alone and was found dead the next day on his farm at Gelantipy, about 50km north of Buchan, in east Gippsland.
